1. Authority to apply

You declare that you are legally authorized to submit this application.

2. Truthfulness and document authenticity

You certify that all submitted information is complete and accurate; uploaded permits, IDs, certificates, and licenses are genuine, current, unaltered, and lawfully obtained. MediSync may request clearer copies, originals, or additional evidence.

3. Verification authorization

You authorize MediSync to verify submitted information through official records and issuing institutions.

MediSync may contact you for clarification and record the results of the verification process.

4. Privacy and proportional collection

MediSync will collect and use only information reasonably needed for identity, eligibility, application review, safety, fraud prevention, and compliance. Processing follows transparency, legitimate purpose, proportionality, and data-minimization principles under applicable Philippine privacy rules.

5. No guarantee of approval

Submission does not guarantee approval. MediSync may reject or place under additional review any incomplete, inconsistent, unverifiable, misleading, or fraudulent application. Approval reflects MediSync's assessment at that time and is not a government certification.

6. Continuing compliance

You agree to maintain valid licenses and permits, keep material information current, notify MediSync of suspension or closure, and cooperate with periodic reverification.

7. Suspension, revocation, and review

MediSync may suspend or revoke a business profile or dashboard if a license expires, material information becomes inaccurate, ownership is disputed, fraud or tampering is suspected, law or platform rules are violated, or continued access creates safety, privacy, or security risk. Except where urgent protective action is needed, affected applicants may request a reasonable review or appeal rather than receiving unexplained permanent removal.

8. Acceptance evidence

When you submit, MediSync records the user and application, this exact document version, your acceptance state, selected application role, stated authority or position, account email, business name, server timestamp, request audit identifier, and privacy-preserving technical fingerprints of relevant network, session, and device signals. Raw passwords, OTPs, session tokens, source IP addresses, and full device headers are not stored in this declaration record.
